Sessions/Tracks

Track 1: Nephrology and Therapeutics

Nephrology is the branch of medicine that deals with the physiology, diagnosis and management of kidney diseases. The kidneys are a pair of organs lying on either side of the spine between the peritoneum and posterior abdominal wall surrounded by muscle, fat and ribs. These are covered by a dense fibrous capsule and irregular connective tissue helping to hold the shape. The kidney section reveals outer cortex and inner medulla, there are columns from the cortex through medulla separating renal papillae and pyramid. Through renal hilum the nerves, vessels serve the kidneys. The kidneys are the vital organs that play a crucial role in maintaining the fluid, electrolyte balance, filtration of food, medicines and toxins from the blood and maintaining hormonal balance and blood pressure in the body. Therapeutics is the branch of science that deals with the treatment for the disease/disorder or care for prevention.

Track 2: Diseases in Nephron

The Nephrotic disease is caused by various conditions that have done harm to your kidneys. Kidney failure is that the last (most severe) stage of chronic uropathy. That is why renal disorder/ nephropathy/ nephrosis failure is also referred to as an end-stage renal disease or ESRD. Diabetes is the most common cause of ESRD. Other health conditions that can aggregate kidney failure include Hypertension, Glomerulonephritis, Autoimmune diseases such as IgA nephropathy and lupus, Cardiovascular disorders and Obesity. Most commonly occurring kidney disorder includes Nephrotic Syndrome  in which your body excretes too much protein in your urine, which is primarily caused by Minimal- Change nephropathy, Membraneous nephropathyFocalglomerulosclerosis and secondarily by system disorders like Diabetes mellitus, AmyloidosisLupus erythematosus causing damage to the clusters of small blood vessels that filter waste and excess water from your blood and thus causing swelling particularly in your feet and ankles, and increase the risk of other disorders.

Track 3: Diagnosis of Kidney Disease

Kidney diseases may be distinguished as acute and chronic diseases and can be diagnosed accordingly. Various procedures and test can be implemented like urine tests, urine output measurements blood tests, Imaging and biopsy to identify the rate and extent of severity. Kidney diagnosis rummages around for the amount of waste merchandise, like creatinine and organic compound, in the blood. Urine tests: Analysing a sample of your excreta to reveal abnormalities and to facilitate the reason behind chronic nephrosis. Imaging tests: to assess the kidneys' structure and size. Biopsy: Collecting kidney tissue for testing.

Track 5: Cardio-renal syndromes

Cardio-renal syndromes refer to a set of disorders that occur in renal/cardiac system in the bilateral direction as a result of acute/ chronic dysfunction of one organ that induces acute/ chronic dysfunction of another organ. Based on pathophysiological mechanism these syndromes are further classified as 5 types, wherein cardiorenal type -I acute cardiac dysfunction (acute heart failure, cardiac shock) induces acute renal dysfunction, type - II chronic cardiac dysfunction (CHF) induces chronic renal dysfunction, reno cardiac type-III acute renal  dysfunction ( tubular necrosis, acute renal failure, metabolic disturbances in uremia)induces acute cardiac dysfunction and chronic renal dysfunction( polycystic syndrome, glomerulonephritis) induces chronic cardiac dysfunction ( Cardiac myopathy). In type V simultaneous dysfunction of both organs lead to metabolic and system disorders ( Sepsis, Cirrhosis, Amyloidosis).

Track 6:  Stones in Kidney and Bladder

Bladder or kidney stones are formed as a result of the accumulation of proteins, crystals or minerals in the urine. These are also referred to as cystoliths or calculus. These are usually formed due to incomplete bladder evacuation and get lodged in urethra or ureter causing severe pain while urinating, hematuria, occasionally vomiting, fever and chills. They often occur in hyperparathyroidismrenal tubular acidosis and dehydration. Commonly diagnosed through symptoms and CT. Ultrasonography, X-ray examination of the abdomen, excretory urography and urine analysis can also be done. These stones may of different types like calcium, cystine, uric acid and struvite stones. Dependingly the treatment varies and different strategies like stone bypass procedures, surgical removal (Shock wave lithotripsy, ureteroscopy, percutaneous nephrolithotomy, endoscopic surgery) and alteration of urine pH may be implemented. Often NSAIDs and opioids are given to relieve the pain associated.

 Track 8: Glomerular Disorders

Glomeruli along with tubules form basic functional units of the kidneys. These glomeruli filter the waste, toxins, fluid in blood retaining the red blood corpuscles and proteins and tubules convert the concentrate into the urine. The disease in glomeruli leads to impaired infiltration causing hematuria and proteinuria leading to fluid retention in the body mostly in feet, face and ankles. These disorders may be of acute or chronic origin mostly caused due to infection, excessive usage of NSAIDs, systemic disorders like diabetes, lupus or idiopathic or sometimes due to autoimmune disorders likeGoodpasture syndrome, IgA nephropathy. Various other disease conditions that lead to glomeruli disorders are glomerulosclerosis, diabetic nephropathy, focal segmental glomerulosclerosis, membranous nephropathy, minimal change disease. Nephrotic and nephritic syndrome are common conditions seen in glomeruli disorders. Comorbidities include Diabetes, Hypertension, Obesity, Age above 65 years. The diagnosis included urinalysis, urine output analysis, serum creatinine analysis Kidney biopsy. Common management includes ACE inhibitors and ARB's to control high blood pressure, corticosteroids, diuretics to manage fluid retention and lifestyle modification.

Track 9: Tubular and Interstitial Diseases

Acute or Chronic inflammation of renal tubules and interstitium causes tubulointerstitial diseases. These are caused as a result of metabolic disturbances, drug allergic reactions ( Lead, Mesalamine, Cardium, Lithium ) prolonged exposure to toxins and disease conditions like multiple myeloma, systemic disorders (SLE, Sarcoidosis) and infections ( caused by Legionella, Brucella, staphylococci, streptococci ) can aggregate the disease condition. These are often associated with painless pyuria, hematuria, fever, rashes. Depending on the symptoms and root cause they are characterized as acute tubular necrosis, tubulointerstitial nephritis, contrast nephropathy, analgesic nephropathy, reflux nephropathy, myeloma kidney. Chronic Tubulointerstitial nephritis can lead to chronic kidney disease and increase the risk of carcinoma.

 Track 10: Drug-Induced Nephrotoxicity

Drug-Induced nephrotoxicity is commonly seen among the renal vulnerable population with underlying disease conditions which aggravate renal toxicity. Most common drugs that cause severe kidney damage include amphotericin, allopurinol, aminoglycosides, ACE inhibitors, ARBs, Benzodiazepines, beta-lactams, cephalosporins, penicillamines, lithium, quinine, statins, sulfonamides, statins, azides, most of the antimicrobials, antineoplastics. These drugs cause renal injury altering the acid- chemical balance, inflammation of nephrons (nephritis), nephrotic syndrome. In some cases, drugs may alter hemodynamics and cause renal scarring. If not diagnosed at an early stage, it may lead to acute kidney failure and may progress to chronic kidney failure.

Track 11: Urinary Tract Infections

Urinary tract infections commonly occur due to bacteria that invade the bladder and referred to as bladder infections. Based on the site of infection they as categorized as lower tract infection and upper tract infection. Symptoms include burning micturition, increased urge to urinate, pain in the abdomen, cloudy urine. Most of these infections are of acute origin and get subsided soon with proper hygienic conditions. Prolonged infections may lead to chronic conditions which require antibiotic therapy. Pyelonephritis- infection of kidneys is an uncommon type of UTI, which is severe and may be fatal. UT infections are more common in females than in males.

Track 14:  HypertensionAssociated with Kidney Diseases

Hypertension is the primary cause for most of the kidney disorders and sometimes it may lead to end-stage renal damage. The increased pressure in the arteries supplying blood to the kidneys make the vessels narrow, weaken or harden causing the insufficient blood supply to kidney leading to malfunction of kidneys, to filter the waste, toxins and fluids from blood. This results in the retention of fluids in the body and thereby increasing blood pressure. The increased blood pressure leads to chronic kidney disease and vice versa. The serum creatinineBlood Urea Nitrogen (BUN) levels are performed to assess the extent of damage and in the evaluation of management. Antihypertensives like ACE inhibitors ( Captopril, Ramipril) Angiotensin receptor blockers (Candesartan, Valsartan) are given to treat hypertension and diuretics like furosemide is considered to treat fluid retention.

Track15: Renal Transplantation& Multiple Organ Transplantation

Renal Transplantationkidney transplantation is the procedure where the healthy kidneys are transplanted to End-Stage Renal Disease patient. Depending on the source of donor this transplantation may be a deceased donor transplant or living donor transplant. This is often termed as Golden standard therapy for ESRD patients. Yet various other factors like compatibility, comorbidities, underlying disease conditions play a vital role in determining the success rate of transplantation

Track 16:  Anatomy & Physiology of Kidney

The kidneys are the excretory organs of the human body, lying in the retroperitoneal sphere on either side of the spine between the peritoneum and abdominal wall. These are the high vascular structures roughly ranging to own fist size. Left Kidney is situated between T12 to L3 vertebrae and the right kidney is slightly down. In males, each kidney weighs about 125g-175g and in females, 115g-155g. They are about 11-14 cm in length, wide 6 cm and 4 cm in thickness, covered by a dense fibrous capsule and irregular connective tissue helps in holding their shape. The renal fat pad around the capsule acts as a shock-absorbing layer. The renal cortex and medulla form the outer and inner regions of kidneys, renal columns radiate from cortex to medulla region, separating the renal pyramids and papillae. Renal Hilum serves as the entry and exit port for nerves, vessels passing through kidneys. Nephrons are the basic structural and functional units of the kidneys, the capillary bed called glomerulus serve as a filter and the filtrate is collected in bowman's capsule encapsulating glomerulus. The arteries and veins serving the kidneys arise from the aorta and drain into superior vena cava respectively. Secondary capillary bed at distal, proximal convoluted tubules and loop of henley forms portal system where the ultrafiltrate is formed, which passes through minor calyces forming major calyces and then to pelvis and ureters. After complete reabsorption and secretion, which helps to maintain fluid, electrolyte and hemostasis balance in the body, the final output urine is formed.
